Nice Form Styling

I’m a stickler for nice forms both in styling and in html structure. This form is probably the best example I have ever seen. It uses the fieldset, legend and label tags properly and does away with the way overused table for layout.

    That’s a nice site – but I can’t seem to find the form – comments have been disable everywhere I look!
